Stephanie Sargent: So, in my experience working with healthcare organizations and practices in general, communication is almost inevitably one of the most frequently contributing factors to serious safety events or adverse events.

Part of the communication problem is people feeling unsafe to speak up. In fact, there’s data that shows that 9-times-out-of-10, a professionals will see another professional making an error and yet they say nothing.
